A silly question, but I am in such situation:
I have the GothNo as the 'title' in the document (hopefully will replace it yet with something making more sense), and I ten to write in 'scenes' or 'sketches', which tend to be separated by asterisks and usually have titles, that remind me of what to write. Except they're tiny, word count wise.
I assume neither the title nor these 'scene' titles count, yes?

Well, you don't have to include those notes if you don't feel it's fair.
However, last year and also for nano, I always had at the bottom of the page a few small paragraphs of notes, more like brief summaries/outlines of the next bunch of scenes. I thought of them as very short versions of the next two chapters. So, when it came to those actual chapters, I just re-wrote them with more words (and not point form). So I replaced those notes with the actual scenes. So at the end of your story, those notes won't be there anyway, right? But while you're writing, you could think of them as just crappy, short versions of scenes that you will rewrite in a better way in the near future. So in that sense, they are a part of your story. But again, if you don't feel it's fair to include them then you don't have to.
With the title~ I'm pretty sure the title would be part of your story, and chapter titles too, since if you were to publish a story those would also be included in it. But I'm not sure if that's how other people feel? I think it would be fair.

He could feel them, bathing inside a water fountain and mating – the same dance ever and ever, only the participants growing ever more annoying and changing – over his head. The water running out of the faucets.
(no comments on my grammar and style, though)
How does that look like? And oh yes, it can get very crappy. The one I posted is rather OK.
